According to Medscape’s handy drug interaction checker…
#1 Tramadol may reinitiate opiate dependence in patients previously addicted to other opiates and may provoke withdrawal symptoms in patients currently addicted to other opiates
#2 Both tramadol and methadone produce sedation and this combo should be avoided
I would avoid the tramadol if at all possible.
I’ve used this website a lot and also been addicted to opiates (as well as tramadol, which is addictive even if not a “true” opiate). Hope this helps. 🙂

If you take methadone regularly and are or may be dependent you should NOT take tramadol. Tramadol is a partial agonist and if someone who is dependent on opiates takes a partial or full agonist the person will go into immediate withdrawal. Basically you would get very, very sick in just a few minutes. It’s usually not dangerous but it’s extremely uncomfortable. For people with certain health problems it could even be dangerous.

If you are not dependent on methadone you will not experience withdrawal symptoms but the Tramadol will basically kick the methadone off the receptors in your brain that they are occupying which would cause the methadone to no longer work for you. These are not drugs that should be mixed. Other partial agonists that should not be mixed with methadone are Ultram, Stadol, Buprenorphine, etc.
