I can’t imagine what to do under those circumstances. Get her arrested would be a start. Heroin use is illegal.

Involve the police, anonymously. There’s special child endangerment/abuse charges that can be brought against women that are pregnant.

I’m not sure – does your state require professionals to report potential abuse. And does your state get actively involved with threats to the unborn.
